Privacy Policy

Introduction
This policy describes how the Studio Dentaire (Vaudreuil-Dorion) website collects, uses, shares and retains the personal information of the people who visit it. It applies to the website only. Information collected at the clinic as part of your care (patient record, X-rays, billing) is governed by the clinic's professional and legal obligations and is handled under separate practices.
It has been written with Québec's Act respecting the protection of personal information in the private sector, as amended by Law 25, in mind. [To be confirmed by legal counsel]
By using the site, you acknowledge that you have read this policy. If you have questions, the contact details of the person responsible are at the end of this document.
Information we collect
The site asks for personal information only when you fill out the appointment request form. The fields are: your name, phone number, email address, whether you are a new or existing patient, your preferred day and time, the general reason for your visit, your preferred language, and your consent to be contacted.
We ask that you not include medical details in the reason-for-visit field: a general description (for example "exam and cleaning" or "tooth pain") is enough. You will be able to explain your situation in detail during the confirmation call or at the clinic.
Only with your consent, the site also collects usage data through analytics tools (see the "Cookies and analytics" section). No user account is created and no financial information is collected by the site.
When you call or write to the clinic, the information you provide is handled under the clinic's practices, not by the website.
How we use it
Form information is used for one purpose only: to reach you to schedule and confirm your appointment, in the language you chose. It is not used for marketing, is neither sold nor rented, and is not used to build a profile.
Analytics data is used only to understand, in aggregate, how the site is used (pages viewed, where visits come from, device types) in order to improve its content and accessibility.
Security
The site is served over an encrypted connection (HTTPS) and the form is processed server-side. Emails sent to the clinic are delivered through a mail service that supports encryption in transit.
No transmission over the Internet is entirely secure, however. If you would rather not use the form, you can always call us at 450 510-1717.
In the event of a confidentiality incident presenting a risk of serious harm, the clinic will take the steps required by law, including notifying the people concerned and the Commission d'accès à l'information du Québec.
Your rights
At any time, you may: request access to the personal information the clinic holds about you; ask that it be corrected if it is inaccurate, incomplete or ambiguous; withdraw your consent to the use of your information or to audience measurement; and, in the cases provided by law, ask that it no longer be disseminated or that it be deleted.
To exercise these rights, contact the person responsible for the protection of personal information (details below). We will respond within the time limits set by law. Proof of identity may be requested to confirm who you are before a request is acted on.
To withdraw your consent to analytics cookies, delete this site's cookies in your browser settings: the banner will be shown again on your next visit and you can decline at that point.
If you believe your rights have not been respected, you may file a complaint with the Commission d'accès à l'information du Québec (www.cai.gouv.qc.ca).
Person responsible for the protection of personal information
As required by law, the clinic has designated a person responsible for the protection of personal information: [Name of the person responsible — to be confirmed]. [To be confirmed by legal counsel]
This person oversees the application of this policy, receives access and correction requests, consent withdrawals and complaints, and coordinates the response to any confidentiality incident.
Changes to this policy
We may update this policy to reflect changes to the site, our practices or the law. The version in force is always the one published on this page, with its update date. If a significant change is made, a notice will be posted on the site.
